Why stateless AI tooling produces compounding operational failure — and what deterministic context governance addresses
AI-assisted engineering tools have matured rapidly at the model layer. The same maturity has not occurred at the operational layer. This paper documents the structural failure modes that emerge when AI models operate without governed, persistent context: context drift, operational drift, release ambiguity, provenance loss, and non-deterministic workflow outcomes. It then introduces the deterministic operational engineering model — append-only provenance, governed context composition, and bounded authority — as the engineering response.
